Can cervical radiculopathy cause pain in shoulder, arm, side of chest and sometimes hand? But no neck pain? Most pain is inner arm. Mostly when I move.
Strange but true: Cervical radiculopathy occasionally can be present without the presence of significant neck pain. That is a little odd when we see it and for that reason deserves to be looked at in detail to make sure something else may not be causing neurological symptoms such as a mass lesion, nutritional deficiency, etc. Emg studies often help answer this question as well. Neck and Neuro: Depends upon your neurological problem as to which doc to see. Physiatrists (pm&r specialists) can often deal with this mixed bag very nicely. Provide therapy, pain management, address spasm/contractures, bracing, etc. Neurologists may also help from the standpoint of nonsurgical care. See below: You may want to try applying moist heat to your neck 2-3 times per day and start a short course of anti-inflammatory such as Advil or Aleve (naproxen). Make sure you have a good lumbar support if you sit in a chair several hours per day. This will help with posture and will reduce stress on the neck. Eval by PM&R: Would be advisable to see a physiatrist. Would consider physical therapy, ice/heat pack, nsaid if no aspirin allergy, and hep program. Also, should be aware of posture and activities that could be worsening problem. So if picking heavy objects should avoid it. Depends: If you have significant compression on the nerve roots based on the MRI scan and your symptoms are limited to the same involved nerve roots, then surgery will help you and is a good idea. All things are: Possible and the fact that you have ongoing neck pain needs to be evaluated to see if your fusion healed and if there are no adjacent level issues in the neck. Cervical medial branch block with DEXA lidocaine gave me 1 day of neck pain relief. After 1day neck pain again. Ok to Proceed to radiofreq ablation?
Be patient. Some times it takes a few weeks for DEXA to get effect. Ablation is a serious procedure with some side effects and no warranties. Rec. See your pain management specialist for medication adjustment (e.g. tramadol, relafen, (nabumetone) Lidocaine 20% patch, etc.)

Go back to basics!: Your profile says you have celiac disease, and so if no trauma is involved at the time of symptoms starting, you need to look at your diet & gut since then. What did you eat? What were you doing (activity, trauma)? There are referred pain patterns from the gut, gallbladder, colon, liver that needs to be investigated further by your doctor.
