What's the cause ?: Cellulitis is basically a visible inflammation (itis) of the skin and soft tissues (red, hot, swollen and tender) . It can be caused by many things including sunburn (radiation), trauma (sprained ankle) or poor vein circulation (stasis dermatitis). It need not be caused by bacteria. We also have to worry about the ligaments which are most commonly injured during an ankle sprain and will not show up on plain film x-rays. Could be serious.: Hemihypertrophy can mean a difference in length of the leg as well as girth of the leg. If you have suddenly noticed a difference in girth with searing calf pain and pitting edema, you need to go to the ER immediately because of the possibility of a blood clot. I believe in playing it safe since failure to treat could have devastating consequences. ...Read more

Not normal: The potential of skin infection is very real especially if tetanus immunity is not up to date. Extremities do take longer to heal but don't risk missing a potential infection. Seek out medical care for an exam since there was a break in the skin and you still have pain. ...Read more

Mgt: The D Dimer and chest radiograph may not be fully accurate in the diagnosis of a thrombosis or embolism. With your symptoms, I would advise having a V/Q scan of your chest and another ultrasound of your lower extremities including upper legs. Other imaging can include a chest CT.
