Recommend a SGA: Second generation antipsychotic (SGA) in combination w/the celexa you are already taking. Regardless of whether or not the intrussive thoughts are secondary to an underlying OCD, major depressive d/o, bipolar d/o, neurodevelopmental d/o, psychotic d/o - all of which can present with intrusive thoughts, and in all such cases, a SGA can be effective. Medication reeval: Your husband is on two antidepressants, abilify (aripiprazole) which is closer to the atypical antipsychotic class and stratera which is chemically very similar to an antidepressant. Probably snri: Snri most likely but everyone is different--some people do better on the older meds some with augmentation etc--if someone in your genetic family has similar sx & is doing well on something that is probably the best thing to try ...Read more

Not stronger: Zoloft is not really stronger than Celexa or lexapro, (escitalopram) but they're not the same exact molecule. All 3 are ssri antidepressants. They differ a little in their activity with certain neuron receptors and reuptake of ne, for instance. If you need a medication from this class, it's worth trying zoloft very gradually. Try CBT: All research comsistently finds that optimal treatment includes good CBT therapy and your active involvement. With or without medication, results are sugnificantly better. Discuss the problem with psychiatrist, he may change the medicine or add another one to make the nausea go away, so her appetite improves. Almost immedicately: The side effect is dose related and improves as soon as the blood levels drop which could be the next day. Of course over time if the depression returns your libido could plummet. Lexapro (escitalopram) may have less sexual dysfunction than Celexa. Young men who are anorgasmic on low doses of SSRI, I wonder about Bipolar disorder and would consider Lamictal as alternate rx ...Read more

It's not, really: Zoloft is not really a "stronger" medication than Lexapro (escitalopram) or celexa. All 3 are selective serotonin reuptake inhibitors. They differ a little in the particular receptor sites they affect -- zoloft may block norepinephrine re-uptake a little more than Celexa and Lexapro (escitalopram) do, but not nearly as potently as paroxetine, duloxetine, and some older tricyclics. ...Read more

Serotonin mishap?: A more gradual taper of the Celexa may have been easier to tolerate. Shouldering one gradually down and the other up usually works better, i.e., Celexa down to 10, and more gradual increase/addition of Zoloft, 25 for 10 days then stop Celexa and increase Zoloft to 50.
After that many years and abrupt cessation you may be experiencing serotonin discontinuation from the Celexa - despite the Zoloft ...Read more

Half and half: There are few definitive anythings when it comes to psychotropic medications. If you're thinking about switching to an SNRI anyway, you can try something that works on both Serotonin and also Norepinephrine, like Effexor (venlafaxine). Agree w/Dr. Buggie: I agree with dr. Buggie about the need for psychiatric evaluation and ongoing treatment. Wellbutrin does not help with anxiety. Paxil (paroxetine) is a reasonable choice, but whether it's right for you I cannot say. You're also on Adderall xr, which can exacerbate anxiety in some. Psychotherapy is an important part of treatment for anxiety also -- but you didn't mention pursuing that. ...Read more
