Few things: This could be caused by nerve irritation stemming from your upper cervical (neck) spine. It could also be related to your mastoid bone. If you have had any recent infections, such as an ear infection, that infection can migrate to your mastoid bone and cause complications. What can cause pain behind right ear that radiates down neck into arm? Pain increases with inhaling and stops when holding breath. No cold symptoms.
Radiculopathy: Pain that radiates down the arm can reflect a cervical radiculopathy (shooting pains due to a pinched nerve in the cervical spine). I'm not sure what to make of the association with breathing, although conceivably a pancoast tumor (a tumor a the top of the lungs) could cause the constellation of symptoms you describe. If you do not have any hearing loss or any pus from the ear canal, the most common cause of pain just behind the ear is inflammation of the attachment of the large neck muscle (SCM) that attaches to the mastoid process.
