I can think of a reason why an incision would be necessary to determine the source, unless there is a suspicion of a tumor causing the swelling. MRI can tell: as long as the correct "sequences" were performed and the right area was imaged. For instance, a knee MR only includes a small area of the leg. Your pain could be below or above that. One type of sequence looks specifically for fluid in the muscle, and it is very sensitive for muscle strains.
