Could be occipital: Occipital neuralgia is a condition that may cause tenderness on the back of the scalp. It is a little different than a tension headache- the source and treatment are different for example. An injury or problem with the neck is a second possible cause. Tension headache can last this long however and may also be the cause.
Different things: Possibly chronic daily use of over the counter medications and not sleeping too well and being quite anxious on top of a headache tendency.
Can anyone tell me what causes chronic tension headache when a piece of scalp feels sore and tender?
Chronic Tension-type: Headache can develop from episodic tension-type headache and the resultant headache is considered a chronic form. The clinical presentation is similar to episodic tth except for the increased frequency of attacks. The headaches are usually bilateral and diffuse and often have tenderness in the cervical muscles as well as the pericranial muscles.

Sound like Sinusitis: Your symptoms are consistent with sinusitis or sinus headaches How long have you had your symptoms? What other symptoms are you having? What makes it better or worse?
