You are mixed up: Normocytic normochromic are terms sued to describe normal red cells. That has no real connection with Lymph node enlargment unless you have Anemia which can certainly be associated with certain diseases of Lymph nodes, like Lymphoma. If your nodes are getting progressively larger, then your doctor needs to find the cause....this may require a biopsy of the lymph node as well as of the bone marrow.

Impossible to say: There are numerous causes for enlarged lymph nodes. Without knowing the results of the testing done in your case, it is impossible to be specific. However, if multiple doctors found no real issue, i would accept that.

More info needed: Depends on where the nodes are and if they have changed. Was a chest x-ray done? What kind of blood work was done?--viral studies? Good luck.

Why can smoking on isotretinoin aka Accutane cause chronic lymphadenopathy ?

Not aware it could: and after doing a review, still can't find that it is associated. Accutane can itself cause swollen lymph glands, and smoking, when it irritates tissues in the mouth and throat can itself cause lymphadenopathy, so there may be a synergistic effect between the two. Accutane however can markedly increase the incidence of lung cancer in those who smoke. Either stop accutane or stop smoking.

My recommendation: 1. ANA, can be nonspecific. Positive usually reflect an autoimmune process such autoimmune hepatitis or SLE, etc. With titer of 1:320, I recommend you see a rheumatologist for further work-up. 2. Hilar and mediastinopathies are concerning. If CT report does raise a concern, you should see a pulmonologist as the first step. Not sure if the two is related or not at this point.

Possibly: The most common cause of eosinophilia in the world is parasitic disease. Exposure can happen when traveling to high-risk countries. Stool samples can be taken for diagnosis. Other causes of eosinophilia include asthma, alergy, drug hypersensitivity, DRESS, connective tissue disorders, and various cancers. See an physician so they can investigate possible causes and perform appropriate workup/labs.

In many cases,: A needle biopsy is not adequate for a diagnosis. There are a fair number of infections as well as other things that can do this.

Many possiblities: Lymph nodes enlarge for many reasons, often because they are fighting inflammation, which is one of their primary functions, but they are also susceptible to primary tumors or metastatic malignancies, non-infectious granulomatous disorders like sarcoidosis and others. In order to determine the cause they may have to be biopsied, although a full evaluation of your health is warranted. Good luck.

What are some possible causes of bulky mediastinal lymphadenopathy(other than cancer) involving subcarinal, bilateral hilar and paratracheal nodes?

Sarcoid: could cause adenopathy in those regions, as well as other granulomatous inflammatory processes. Infectious processes such as histoplasmosis could do it. Certain chronic exposures such as berylliosis and silicosis could cause mediastinal adenopathy, but usually the history and other findings make those diagnoses more obvious. Unfortunately, lymphoma is the major consideration in most cases.
