POSSIBLY: Certainly, herpes zoster (shingles) can present as a severe pain along a dermatome, and can be associated with a rash. I don't have enough information to provide a good answer. How long has the pain been present? Any history of injury? What worsens the pain? What makes it better? What diagnosis have you been given? How severe is the pain? What lab tests have been done?

Possible: Endometriosis may be the cause of flank pain, but there are other reasons to consider as well. If pain is worse during menses makes endometriosis more likely as a cause. If so you will need antibiotics at a minimum with guided placement of a drain, or better yet surgical removal of the affected tube.
