Fertility in 19th C: I don't believe there's much real data to answer , but case reports in lancet as far back as civil war support that causes were probably similar to now: ovulatory, tubal, endometriosis. There's a fascinating case report of a surgeon cannulating proximal fallopian tube with a whalebone in 1898! delayed child-bearing of course was less common.
