People often use the word "cramp" to mean any crampy pain, but to a doctor, a cramp is a painful contraction (squeezing) of a muscle that moves a joint. For example, a cramp in the calf will cause the foot to move down to a tiptoe position, while a cramp in the foot may cause the toes to move up, down, in, or out. It is important to know whether the cramps are true cramps or pain without true cramps.
Restless limb syndrome, for example, does not cause true cramps, although for some people the pain feels quite achy.
True nocturnal (night time) lower limb cramps are a fairly common, and usually benign (harmless) problem. Some cases are linked to medications, such as diuretics ("water pills" or "fluid pills") or statins (cholesterol lowering pills), but no one really has a good explanation for what causes nighttime leg cramps otherwise. We know that they can happen during the later stages of pregnancy, are more common in teenagers and old folks, and seem to run in some families.
Because we don't exactly know what causes nighttime leg cramps, there isn't a one-size-fits-all cure. A lot of experts agree that light exercise at night (most mention riding a stationary bike at low speed and low resistance) and stretching the low back and low limbs are effective. Avoiding dehydration may also help. Supplements such as vitamin d, magnesium, and coenzyme q10 may help some people.
