Probably from biting: You probably have chronic inflammation or a "granuloma" - a fancy name for a type of scar from biting your lip this is pretty common. It usually results from a nervous tick or habit. First thing is try to stop and see it goes down. This is harder than it sounds and you will have to pay attention and find something else to do.
