Melasma?: Melasma or changes in facial coloring is related to hormonal changes. Since you are post-menopausal age, are you taking hormone or estrogen replacement therapy? Are the changes recent or long term? Talk with your GYN or dermatologist about options such as creams or laser therapy. If taking hormonal therapy, the changes may be chronic as long as you take estrogen/progesterone.

Dark spot on head: It could certainly be melasma - if it is cosmetically unappealing - then there are several different treatments available to lighten the area. To be certain this is what it is a biopsy might be in order Good luck

Melasma: Melasma is a common acquired increase in pigmentation occuring exclusively in sun-exposed areas that is typically brownish in color. It tends to be prominent (but not exclusively) among latinos and asians, and more apparent during and after periods of sun exposure. It mostly arises from pregnancy (chloasma) and oral contraceptives, but can also be associated with endocrine dysfunction, genetic factors, certain medications, nutritional deficiencies, among other things.

Melasma?: Melasma of the face is often in a butterfly pattern but may be spotty. If you only have one spot and it is growing, you need to see a dermatologist and get a biopsy.

Hormone related...: Melasma, chloasma, and the "mask of pregnancy" are all terms for facial pigmentation that is increased by the use of estrogen (in pregnancy, of course, the estrogen comes from the placenta). See a dermatologist: ???? TCA= tricyclic antidepressant???? Skin changes probably not related. Unfortunately you omit any details. Imipramine (which do you take?) may cause some subtle change of skin color in about 1 out of 20 people.

Various treatments: Melasma can be frustrating to have and a challenge to treat. Daily sun protection is important. Often the best results may occur by using a combination of treatments. These include topical treatments, peels and sometimes laser. Lasers can be helpful, but I recommend treatment with low energies, as higher energies can sometimes stimulate pigment worsening. Even when tx helps, recurrence is common.

See a dermatologist: Details matter. You fail to specify any detail about the "medicine". Prescription creams are more effective than laser for melasma. Typical approach is to use sunscreen every AM. At night she would apply a cream combining a retinoic acid, a low strength steroid, a hydroquinone, maybe others. Expect to do this for several months. It is less costly and more effective than laser.

Probably safe: Long-term side effects not an issue. But do not expect much benefit. There are products which are very effective. A dermatologist will help you.

Chronic: Chronic melasma is difficult to treat. I recommend elta md sunscreen (zinc oxide based), obagi (a skin care system that contains skin fading creams), retin-a, and a series of vitalize peels. Read more about obagi, vitalize peel, and elta md at the link below. I hope that helps a little!

Rosacea, I think?: Given your description, looks like you have rosacea. You amy want to consider seeing another doctor for a second opinion. You may also have an auto immune condition so for sure you should get another opinion. Melasma is very hard to treat.
