Is a clot, a bakers cyst is another that comes to mind. A clot is serious if present and needs to be dealt with immediately. The question is did you already have he ultrasound or not? It's impossible to tell whether you have a sprain or a fracture without an x-ray, so yes, you'll need to see someone to have it evaluated and x-rayed. In the meantime, I would not apply any heat, as heat will increase the swelling. Ice is ok, but now at 3 days later, it's doubtful ice will help much. Also, I was lay off the ibuprofen. If it is fractured, Ibuprofen delays and hinders fracture healing. Take tylenol (acetaminophen) for pain, but go see a doctor, either a podiatrist or orthopedic surgeon.
