May be other factors: People can have a viral infection and or mono and test positive for strep at the same time. Strep are universally killed by the penicillin group of abx while occasional resistance to erythromycin/zithromax have been reported. If you have a dual infection chenging abx won't help. It is not 100%, but still the "standard of care." it is sometime not used because of penicillin allergies or because of younger age (adolescent/teen-more difficult with schedule--school/sports etc) due to required frequency of use --4x/day for 10 days.
