Immersion: The skin is soaked and needs to dry out. It could laso be reaction to color of shoe. A peeling cream containing urea combined with a topical steroid will remove the dead skin and calm the inflammation. Fungus can develop under these conditions especially between the toes and at the end of the nails. If it begins to deelop a topical antifungal cream should e added.
Not many: No greater risk than swimming in a lake. I wouldn't worry unless you have corneal disease or are prone to corneal or conjunctival problems.

Two possibilities: Either irritation from the pants rubbing on his skin or "jock itch" which is a fungal infection caused by the fungus, tinea cruris. It's not serious and is readily treatable with a combination cream which contains a steroid and an antifungal medication; one such med is Lotrisone (clotrimazole and betamethasone) cream (it's available in its generic also). Recheck: Sutures may have loosened or broken open- have the foot reexamined by the physician who sutured the wound. Apply a sterile pressure dressing over the wound and have it rechecked asap.
