They can be: Lack of oxygen can be a factor in all of your symptoms. Is the chronic sinusitis due to obstructions (polyps) or a deviated septum?
Has your sleep apnea been diagnosed by a witnessed overnight sleep centered study? If so have you been advised to wear a cpap?

MRI very sensitive: Just because you have something in the sinus, it does not mean it is the cause of your symptoms. Retention cysts are common. Sleep apnea and DM2: Your doctor may be correct about your sleep apnea but you must be referred to a specialist that can help you.
Regarding type 2 diabetes - please stop that term.
Type 2 diabetes can be totally removed once you have a good diet and good exercise. Please have your doctor refer you to a dietitian.

Call cardiologist: With severe cardiomyopathy and a crt-d, you can't assume a simple or harmless cause for any kind of dizziness. Osa occurs when airway is blocked during sleep, muscles flaccid/paralyzed. When co2 is too high, brain enters crisis mode, lightens the level of sleep to permit active muscle control and permit a big gasp to replenish o2, then back to deep sleep to do it again.
