No: A primary ovarian cancer is hard to dx because mostly asymptomatic. When it spreads, it induces ascites from omentum. Then spreads to bowel where GI symptoms arise. Unlikely,: Your age and presentation suggest many other likely diagnosis. Incidentally, how did you know that your estrogen is low? Cramps in 39 y female could are likely to be due to menstrual cycles. When does cramping occur? Cyst, infection, review of other systems... Cancer is low on the list. The differential is wide open.
