Signs of kidney dama: There are no classic signs of kidney damage, kidney damage can occur silently. Recurrent uti's, specially pyelonephritis can lead to chronic kidney damage, long standing obstruction of the urinary tract, urinary reflux into the kidneys, congenital and hereditary abnormalities like polycystic kidneys, congenital obstruction (upj), glomerulonephritis.

Metformin: Lots of missing information. You are taking lots of meds - do you really need them? Are you overweight? If so you need to see a dietitian to help get your weight down. Metformin per se should not cause kidney damage.

It's because...: The elderly with multiple medical conditions such as poor kidney function, hypertension, diabetes, etc. Tend to be more vulnerable to sudden BP drop resulting in further kidney damage by poor circulation and ischemia of kidneys. But its degree may vary and be unpredictable. For the time being, try not standing up quickly, keep up more adaptable exercise, and adjust medication... Talk with doctors.

Reflux: I depends on the concentration of the medicine. Usually zantac (ranitidine) liquid is dispensed as 15 mg in 1 ml. If your baby weights 8.5 lbs, the dose should be more around 1 ml twice a day but you should check with your doctor to verify the correct dosing. If the baby is not getting better, you may want to visit a gastroenterologist (GI) specialist.

Glucomannan.: Not one knows but remember they can and do put what every they want in any supplement. Lead, mercury licorice. All of which can damage your kidney.
