Cervical radiculopathy: Sounds like it could be related to a cervical radiculoapathy. Radiculopathy symptoms can wax and wane over the years causing a variety symptoms including numbness, pain, and weakness. Consider MRI of cervical spine and electrodiagnostic studies of the upper extremities. While a cervical radiculopathy can caused muscle spasms, you typically don't see 'spasticity'. Radiculopathy would be a lower motor neuron condition as opposed to upper motor neuron. Does the patient have increased reflexes and positive Hoffman's test?
