One Surgery: There are no current excellent studies that would indicate a better outcome success rate of one over the other. Generally, if the procedure can be done with one surgery rather than two or more it will preserve more bone structure overall. Dental implant: The problem with having an unrestored dental implant is that, depending on how long you wait, the teeth on either side of the implant may drift towards the implant site. This would necessitate orthodontic treatment to re-align the teeth prior to restoring the implant. Depends on area: Sometimes the dental implant can be placed at the time of extraction if the site has sufficient bone to engage an implant and there is no infection and sometimes the extraction site has to be grafted with bone (and connective tissue as well) and the implant placed after the site has healed. Each case is decided on an individual basis on what is best for the patient and will have the best results! ...Read more

Variable: Many different possibilities. We often extract the tooth and place the implant at the same time. We will also place a temporary tooth on the implant during this procedure in many cases. Usually 12 - 16 weeks later we can make impressions for the final restoration. So, using this scenario, immediate placement and 4 months to restoration. When growth stops: The short answer is yes! there is s minimum age, and it may be as early as eighteen, but i can not tell you the number. What determines the suitability of a young patient for implant placement in the mouth is time when they reach the end of their growth spurts. Girls are usually a little earlier that boys. We used to talk a lot about taking x-rays of the wrists and making a determination about when the growth centers fuse, (epiphyseal fusion), but a more accurate determinant now is the lateral cephalogram. On of these x-rays taken at a time when your son's shoe size stop increasing, and then another 4 - 6 months later, and if superimposition show s complete overlap and now growth he will then be ready for the implant phase of treatment. Now in the mean time you want to maintain that space where the tooth is missing. I recommend either a removable partial flipper for a just a few years , or if the relationship between the upper and lower teeth permits you could bond a false tooth between the teeth on either side of the space.
