He needs a physician: Acyclovir can cause renal damage. As your boyfriend already has kidney disease, the dose of Acyclovir needs to be based on his renal function. Most importantly; does he really need it? Under any circumstances, this kind of problem can't be addressed here any further.

No: Cold sore, or oral herpes, will not cause low back pain. Initial infection with herpes can cause fevers, but back pain and fevers are more likely to be due to something else.

Probably not herpes: Herpes rarely if ever causes lymph node inflammation without an obvious outbreak of genital area blisters, sores, etc. Even then, lymph node involvement is rare in recurrent herpes, only during the first infection. Finally, genital herpes doesn't cause back pain, and rarely recurs as often once a month -- usually no more often than every 2-3 months. Discuss w/ your doctor if the problem continues.

Herpes: SKIN: Herpes is a skin problem. The virus lives in the nerves that innervate the skin where an infection occurred. This is not going to affect other areas of the body such as your kidneys and urinary tract.

Use a condom: If you do not have the herpes virus already I would be very cautious. I would definately abstain during acute outbreaks and wear a condom at other times. Remember any hing that can make a person sick will make a transplant patient more sick.

Unrelated: The kidney failure is unrelated and needs assessment. If your kidney function is more than 60% not you likely are ok. If less, given your age, it needs evaluation. The genital herpes is unrelated, but treatment doses will need to be adjusted for degree of kidney function to avoid kidney injury from the meds.

Maybe HSV, not VZV: Shingles rarely recurs so frequently, but HSV2 (genital herpes) can do so, with recurrences anywhere from waist to upper thighs ("boxer shorts" area). 1) Retest next outbreak to confirm varicella zoster virus (VZV), and also PCR test for HSV. 2) Have an HSV IgG antibody test, expecting maybe positive for HSV2. 3) Continue valacylovir to prevent recurrences, whatever the cause. The drugs is entirely nontoxic, no reason not to take it as long as necessary. Good luck!

I have been getting blistery sores on my lower back, whenever I eat tomatoes, or citris foods. Is this herpes or cold sores?

Could be herpes: I have heard of citrus type foods triggering a herpes outbreak. You can also get skin irritation from citric acid if it touches your skin and then you are out in the sun. However, it could all just be a coincidence and your rash could be many other things. There is medication for it if it is herpes, so may worth it to get checked. You can always take a pic of it and show it to your doc later.

Low risk, no worries: Herpes doesn't cause low back pain or "itch on butt". The chance of HSV2 from one episode of unprotected vaginal sex with infected partner is around 1 in a thousand. With condom protection, it's still lower. Don't worry about this. Also look at it this way: If you've had sex with say 20 different women, you can assume 3-10 of them had HSV2. The one partner you knew about isn't any more risky.

This isn't herpes: For initial herpes, there usually is no tingling before sores appear. Tingling is mainly a symptom of recurrent herpes; in that case, it lasts only a day or two before the outbreak itself shows up. Your symptoms don't sound at all like herpes, which doesn't cause tingling without sores and rarely if ever causes low back pain. See doctor if symptoms continue, but no worries about herpes. Herpes: This does not sound like herpes, and a blood test for HSV 1 and 2 IgM and IgG can help clarify that. Herpes can occur in the skin of the lower back, usually as a cluster of inflamed vesicles which scab over then heal.

Get tested: Lumps, bumps, blisters and other skin abnormalities on the genitals need to be evaluated. An examination will be needed to know the cause and treatment. My first concern is to rule out herpes but there are many other things that can cause these symptoms
