Bipolar Disorder (Definition)

Also known as manic-depressive illness, bipolar disorder is a brain disorder that in its most severe form causes dramatic shifts in mood, from mania to depression. Symptoms of mania include markedly elevated mood and energy, reduced sleep and a reduced need for sleep, racing thoughts, and grandiose ideas. When mood changes are severe, psychotic ideation may be present. Symptoms of depression can include low energy, feeling sad, feelings of guilt and hopelessness that are out of proportion to the patient's actual situation, low motivation, inability to experience pleasure, and suicidal ideation. A possible combo: The doses depend on your response to the medications and tolerance of any associated adverse effects. It also depends on what other medications you may be taking, particularly other mood stabilizers. ...Read more

First question...: Are you treated by a psychiatrist? Fluoxetine along might not be the optimal choice for treatment of schizoaffective disorder. It could trigger mania/ hypomania. I would encourage working on that issues first and then addressing selective mutism. ...Read more

The doc prescribing: Is the one you should speak to now to voice your concerns first most. And maybe they can help you find a different psychiatrist with a shorter waiting time. How long does it take for 20mg of Prozac (fluoxetine) to reach it's full affect...Meaning how long should a patient/doctor wait to decide whether to adjust dosage?

Depends: There are different strategies for this. In some cases it may be best to push the dose up as high as can be tolerated without significant side effects to maximize the chance of a good response. Once a dose is reached, it usually takes at least a month for most of the effect to take hold (and improvement can continue for 12 weeks or more). ...Read more

Cannot diagnose: It is difficult to know whether the panic/manic episode was causally related to the Prednisone dose, and if it was, whether it was psychological or organic. Prednisone in that dose can produce transient changes in state of mind. Suggest you check with your therapist. ...Read more

Sounds like: Sounds like this patient's psychiatrist would like to see the patient again soon, because Prozac (fluoxetine) + off-label use of Gabapentin does not seem to be having its desired effect. Hopefully this patient is also in psychotherapy as well -- many ways to deal with anxiety can be learned there. ...Read more

Daughter was diagnosed with bipolar 2. Two weeks ago. She is 12.She was put on 5mg Abilify (aripiprazole) 10mg of Prozac at bedtime. Are these a good mix.

Caution, w/ this mix: Antipsychotics can be life saving medications that give a person the ability to work and function in society. If you have a full blown mania it is essential that you try multiple antipsychotics before giving up. Abilify (aripiprazole) is often effective.
