You need evaluation: I have no idea what the pain is, or why drinking alcohol makes it better, but you need to talk to your doctor. Hypertension in a 22 year old man is distinctly abnormal, and if your doctor has not tested you for more unusual causes of hypertension (coarctation of the aorta, primary hyperaldosteronism, renal artery stenosis and others), you should be tested for these.
