Life is not exact...: Maybe, if a cycle is longer than 31 days. If a woman ovulates 2 weeks after the start of her last period, every cycle, then 3 weeks after the start of her last period would be too late to get pregnant. However, if she ovulates a few days later occasionally (maybe 2 weeks and 5 days) after the start of her period, then her egg may still be fertile at 21 days after her lmp. Days are estimates.

Unlikely: It's unlikely that without penetration in the vagina a woman would become pregnant. Also, the fact that the woman was still bleeding, puts her at a time in her cycle when it is unlikely that she will get pregnant. And lastly, since there was no ejactulate, also unlikely. Thus, not zero, but very close to zero.

Not exactly: Women can get pregnant when they are bleeding especially when the bleeding is not their normal period. Not all vaginal blleeding means they are on their menstrual cycle. Ovulation occurs 14 days before the cycle starts and this is the optimal time to get pregnant. When a true menstrual cycle comes it means ovulation did not occur. Having sex during that time would be low risk for pregnancy.

If she were pregnant, and replaced the lining of her uterus, the pregnancy would be lost too. Pregnant women do get vaginal bleeding sometimes, but it is not a menstrual period (even if it sort of looks like one).
