Recurrence rate: The most recent literature is putting recurrence rate between 5-15%. The risk is even higher if the initial abruption was considered to be severe. When the abruption is severe enough to kill the fetus, as in your case, there is a 7% incidence of the same outcome in a future pregnancy, unfortunately. There aren't any studies that demonstrate interventions lower the risk. I wish you the best.

Perinatal loss: Women vary in the time that they take to grief after a stillbirth. Some women want to try again as soon as possible, while others wait for months to try again. Since we're all different, you have to be the judge of when you're ready. You might try counseling to help you.

Good question: Status of evaluation for stillbirth ?fetal autopsy ?placental evaluation ?karyotype ?indirect coombs’ ?serologic test for syphilis ?screen for fetal-maternal hemorrhage (kliehauer-betke or other) ?toxicology screen ?thorough medical and obstetric history ?parvovirus serology ?lupus anticoagulant screen ?anticardiolipin antibodies ?factor v leiden mutation.. These are just a few of the many tests.

Difficult to say: It would depend on why the "placenta stopped working". Was there any testing done? Any evidence of diabetes, uncontrolled blood pressure, pre-eclampsia, blood clotting disorder, etc? Having more information would be helpful in answering that question. None-the less extra vigilance during a subsequent pregnancy is required. Early testing, ultrasounds, etc. Good Luck!

So sorry : ... That she has had to go through such a difficult year. Before trying again, it might be helpful to see a high risk doctor to try to find out why she's having so much trouble. Depending on the reason, there may be things she can do before she gets pregnant to help have a good outcome.

Discuss with doctor: The answer depends on the reason for the stillbirth and the underlying cause. If no cause is known then there is likely no benefit to waiting. Schedule a visit with the doctor who cared for you who knows you history to clarify the plan.

Stillbirth: The pain will probably never go away. It may fade somewhat over time, but you will always feel the emptiness from the loss. The next pregnancy will be high stress even if everything goes according to plan. So my advice would be to wait until emotionally and pschologically you are ready. For some patients this might be a few months, but for most it will be closer to a year.

Sex after stillbirth: Some of this depends on how far along you were at the time of your loss. It's best to check in with the OB who cared for you. But generally somewhere in the range of 2-6 weeks would be typical.

Abruption chances: First of all, I am sorry for your loss. The risk for a recurrent complicated placental abruption in your next pregnancy is about 33/1000. Special surveillance would need to be undertaken about 3 months prior to the gestational age of the baby you lost due to the first placental abruption. Best of luck to you.

I am not aware of : Folic acid increasing the chance of twins, but it will reduce the chances of a specific birth defect, called neural tube defect, which occurs before the woman even knows she is pregnant. That is why prenatal vitamins are important for any woman trying to become pregnant.

IF you were my pt: I would induce u at 39 weeks or earlier , if earlier u must have an amnio but since you are 38.5 i would induce u right at 39 weeks, as the ideal time to deliver is 39-40 weeks and i would be doing nsts and bpps each week until u deliver, indcucion is safe and easy and effective when done properly, almost all of my pts want to be induced, please call your OB to discuss and get it scheduled.

Menstrual : Menstrual cycles can vary at any given time. Many factors contribute to these changes such as stress, anxiety, weight fluctuations, prescription medications, and hormonal changes etc. Like i stated this is normal so no need to worry. Take good care.

No, of course not: Women are free to conceive or not to conceive. Women are free to do whatever they want with their own bodies. If not wanting to conceive, and wanting to continue having sex, just visit the ob-gyn and get some really effective birth control. One can also use condoms in addition to a method from the ob-gyn, for maximum protection. Vasectomy for the husband is an option, but rather permanent.

It varies: If this has affected you, I am sorry for your loss. The death of a child before or after birth will strain the relationship in ways that do not make sense. Now is a time to work hard to reaffirm your love for one another. Ask your physician about support groups; there is no one-size-fits-all answer.

About 1 in 16: Roughly 1 in 4 pregnancies ends sadly. So one in 1/4v1/4=1/16. A medical school classmate's wife had 4 pregnancies end too early. They later had 4 healthy kids.
