Maybe not: If the tendon is mostly intact and there is no gap you could try conservative care. Was this an acute injury? If so better prognosis for success without surgery. Would need 6-8 weeks of protection from sudden jerking force ( no heavy resistance). Needs to be watched and should steadily improve. If not, may need surgery. ...Read more

Depends: Near the distal biceps is the median nerve. Injury to that area can potentially injure that nerve or swelling puts pressure on that nerve.
