U Need Upper GI: By the sound of your symptoms, it's possible that the hiatal hernia has returned. I advise you to return to your surgeon. They will order an upper GI to look more carefully at your "wrap" and see where your stomach is in relation to your diaphragm. I hope it's a false alarm but, worst case scenario, it's fixable. Benefit vs. Risk: How'd you even know that you have a HH & then get so much info? Did you ever have symptoms like heartburn, nausea, vomiting, stomach bleeding / ulcers? While 4 cm is a generous size, if you're not having symptoms, then weigh any (future) benefit of surgery with the risks of surgery (anesthesia, bleeding, infection, complications) which go up with age. Esophageal spasms could easily be a cause of your symptoms, and this can be determined with an test called manometry, which involves a tube in your nose for 15' or so. Potentially: Use of acid blockers could help relieve some symptoms of heart burn. If your chest pain is due to the orientation of the stomach in the chest then nothing will help until the mechanical problem gets addressed with surgery.
