Primary hyperpara: A PTH of 41 with a concurrently high calcium of 10.3 is not normal. Pth and calcium level correlate (inversely) very closely with each other. If ur calcium is high at 10.3, there shouldn't be any PTH around & the level should drop to 5 or 10 but not 41. Need endo. W/U ,,,: You need an endocrine work-up to be sure you don't have an issue with your parathyroids, and for your thyroid. Your TSH is high, as is your calcium, and you are on a high protein diet. These can mean a parathyroid adenoma, and hypothyroidism at least. Probably not: We would need to take a history to assess other medications in use as well as perform a physical exam to assess the state of hydration. The high bun/creat ratio and the slightly high Albumin suggest volume depletion. A urinalysis would be helpful in screening this patient for renal disease. High Urine P/C: Your u protein- creat ratio means you are spilling protein (p) in your urine. You now have good kidney function. See a physician to get a 24 hour urine for a creatinine clearance and protein to quantify the amount of p you have. You will need a drug like ramipril-an ace-inhibitor, or benicar, (olmesartan) an angiotensin receptor blocker to reduce the p you have. If this done, you will protect your kidneys. ...Read more
