Ok to be fearful: If you've been given the OK, just start slow with rehab and strengthening program. It will feel weak in the neck initially and even be sore, don't worry about that it is normal. Likely: Stiffness and muscle weakness after the brace came off. Slowly return to activities and stretch progressively more and more each day. Fear is a normal reaction after something like this. If it hurts all the time and doesn't improve maybe meeting with a spine specialist that addresses chronic facet pain is in order. Possibly?: You didn't say how you got your cervical fracture, but I am sure some trauma was involved. If you fell, even if you didn't hit your head, the jarring could be contributing to your buzzing. Maybe your injury is more severe then expected. Get it checked out by your doctor. ...Read more

Speak with doctor: The extension range may take time to get back if you are supposed to limit it while the fracture healed. Will take constant therapy for a few months at least. Frozen Shoulder also requires constant range of motion, therapy and maybe even a cortisone injection or manipulation under anesthesia to help with this. Facet fractures: Stable fractures of the cervical spine often heal in the first 3 months, and this is a reasonable time for immobilization. Once pain subsides, flexion/extension xrays will likely be taken to make sure no instability exists from ligament injury that would necessitate surgery despite bony healing. C7 Facet Fracture: Most single side facet fractures are stable and heal in two to three months. If you are developing pain in the triceps and back of forearm after 8 weeks, this is indicative of irritation to the C7 nerve root. I'm sorry you're having trouble getting relief of your pain. Based on your description, you have facet mediated pain most likely from facet arthropathy (arthritis). It sounds as though you have exhausted many of the non-operative options available for your problem. Some other options (you may or may not have tried already) include acupuncture, physical therapy, oral medications, and lumbar traction. None of these methods are a "cure-all" but they may give you enough relief to make the situation tolerable and avoid surgery.
If you fail all of these measures and continue to have pain that interferes with your quality of life you might consider surgery. There are some newer techniques that have not stood the test of time (facet joint replacement) but the gold standard is lumbar fusion. It is reliable surgery in the properly indicated patient. You would need to discuss the risks, benefits, and alternatives with your surgeon, but don't expect 100% relief of pain. If you can accept 75% improvement in your pain condition, that would be a reasonable expectation.

This is a common place to develop arthritis which causes marrow edema when the cartilage gets too thin or is worn out and bare bone is exposed there. Bone scan...: Pars fracture typically shows a "scotty dog" on oblique radiograph. Sometimes fracture can show up on bone scan before it shows up on x-ray but it sounds like your injury was a while ago. Have you had a nuclear medicine bone scan or asked about having one? All these factors help determine the significance of any radiographic abnormality. Is there instability?: One consideration is to do flexing and extending xrays. If there is motion that would explain the persistent pain. The MRI can show a stress fracture but sometimes it is hard to tell due to motion artifact. Given the lack of agreement I would bite the bullet and get a SPECT bone scan. If negative then push forward with core strengthening with a physiatrist and physical therapist. If it is a stress fracture and it is not healing you could consider a bone stimulator as an off label treatment.
