Poor correlation: Hi. Oral cortisol (aka hydrocortisone) does not reliably affect ACTH secretion nor aldosterone levels. That is somewhat unfortunate for those of us treated adrenal insufficiency, since determination of proper dose is based solely on clinical response, and not labs. Interesting question: I have not found any publications specifically studying this question. Antipsychotics and Prozac (fluoxetine) can affect prolactin, potentially changing sex hormone levels. This may or may not relate to shbg levels, though. What time of day?: Hi. they may be normal. The TSH could be a sign of early, subclinical hypothyroidism (getting TPO and thyroglobulin antibodies and free T4 may help the assessment). Due to diurnal variation, if the blood was drawn around 8 AM, the cortisol would be normal; very abnormal if drawn late at night. The prolactin, depending on the lab's normal range may be normal or slightly elevated. ...Read more
