It is very difficult to diagnose since all the underlying dental (mucosal change, sore, salivary duct problems, etc...) and medical reasons (sjogren syndrome, diabetes, hormonal, etc...) has to be ruled out. Dry mouth: Loss of saliva, sometimes from medications, sometimes as a result of aging, can cause the painful burning mouth syndrome. There are also auto immune diseases, such a sjogrens syndrome that is genetic that has the same result.
