Happens: It happens because of the pressure of the writing instrument. You may actually have a callus there. The only thing you can do is try to hold your pen/pencil very gently but odds are, the bump will remain. It's harmless.

Without a photo...: Hard to say. Posting a photo could help with the diagnosis. Also, how long has it been there? Any other bumps anywhere on him or on anyone else? Sounds like it could be a simple wart, especially since you say it's "pure skin." Unfortunately, there's not proven treatment for warts; most go away on their own over time.
