Could be a local infection, especially if sensitive to touch.

Son is 8 weeks. At his 1mth check they noticed a water bubble looking cyst on the head of his penis. Now his penis head is covered with a blister.
This child needs attention now. Take the child to pediatrician or family doc tomorrow. If not available go to minute clinic. This child needs to be seen to referred to derm as soon as possible. Might consider that the child might be allergic to the disposable diapers. Would consider putting the child in cloth diapers for now.

Less likely: Although, stress is a reason for itchiness, but it would probably be expected to cause more extensive itchiness than just the ear lobes.
