Tennis elbow?: Might be inflammation at the origin site of the extensor tendons on the lateral epicondyle of the elbow. Ultrasound and xray would be normal; an MRI would likely show some edema/swelling there in the form of increased t2 signal, but this expensive test may not be necessary. Over with your surgeon, as all of us have our own postop routines that we follow. Your surgeon is the only one who knows the type of repair, the quality of your tissues, and the repair he obtained.
Anyway 3 days is too short a time that pain should subside. Generally pain from the incision itself will last for a minimum of 2 weeks or so.

If it is a tendon: It may take months to heal, provided you have adequate brace to support it while it does. Tendons are notoriously slow things to heal because of the poor blood supply to them.
