If he can stand: it is 99% likely it is only an ankle sprain. The common ankle sprain has immediate swelling pain, sometimes a pop and tenderness on the front and outside of the ankle. Contusion injury: THe trauma from being hit/kicked on ankle resulted in a bruise. Swelling and bruising form such injuries can last for one to two months. Need to go to ED: If you fell 20 feet you should be checked over by a doctor and it does sound as if you have broken you ankle. I would recommend that you go to hospital now. ...Read more

RICE: The best thing is rest, ice elevation and compression with Ibuprofen 200-400 mg every 6 hours. Bruising is normal. Get an over the counter supportive brace (not a slip on) and wear that for a few weeks to protect it. 2-3 weeks after injury is a critical period it needs to be protected because it doesn't hurt and you are more likely to re-sprain it.
