Syndrome: You may have a particular syndrome causing this. See your dermatologist.
Fibrocystic changes: You could have fibrocystic changes. This is the most common cause of breast pain. It can be aggravated by caffeine, stress, menstrual cycle and hormonal supplements. If you are over 40, then you should have a mammogram and ultrasound. Please get evaluated by a breast specialist. This can be normal for some women.

Estrogen: The estrogen level increases during the cycle until just before your period when it falls, then starts increasing again as you start the next cycle. By mid-cycle (the time frame that you note - 15 days before your period) your estrogen level is high enough to stimulate the breast tissue making the breasts tender. For many women a vitamin E supplement will decrease or stop that tenderness.
I have reactive axillary lymph nodes under both my arms, did 2 scans showed reactive. Its tender mostly when menstruating? I have it 10yr now?
Lymph nodes: Reactive lymph nodes are doing their jobs. Our lymph system helps to fight off infections. As long as they are reactive with benign characteristics then you have nothing to worry about.
Vaginal entrance is tender during menstration. Especially if the area is dry it hurts to rub there. There are no cuts there as I have checked.

Foot Swelling: Foot swelling could be from inadequate venous return, kidney issues, inactivity, and many other mechanical foot issues. Definitely work on the kidney issues you have, and also spend time elevating your feet, doing hourly ankle circles, get OTC support hose, and if inactive, try to walk more. See a podiatrist, if the swelling and pain do not get better with this to see if you have an actual injury

CKD: Chronic GFR of 63 is ckd stage ii. Ct could be normal and does not rule out chronic kidney disease.

Probably not related: Some common caused of breast pain are hormonal changes, weight changes, caffeine intake, stress, exercise changes, ill fitting bras, cysts and others. An examination is the next step. It is not likely that breast tenderness is related to headaches. A primary care doctor can help being he evaluation of headache.
