Many things: It's hard to do dermatology without being able to see the spots. I suggest seeing a doctor for this. In addition to seeing the spots, the doctor will also look at your med list, what you do with your hands, any health problems you have that my be contributing, etc.

Brown spots on hand: Can be vitamin deficiency Vit C, B12, anemia, liver dysfunction but most likely sun/age but usually that is after 40- unless you spend a lot of time in the sun. Dermatology: Evaluation is necessary: this sounds like possible photo- damage from UV exposure- best to get it checked.

I would encourage you to make an appointment with a family practice doctor or a dermatologist. At your young age, I would not generally think that you have a serious skin condition. Probably no STD: Syphilis can cause a skin rash that involves the palms and soles, but unlikely to cause burning; and syphilis is rare in women in your part of the country. Probably this is something other than syphilis or any other STD. But STDs wouldn't be on your mind if you weren't at risk, so get tested. I am not certain: Exactly what you are asking and don't know whether these spots and dots are flat, raised, painful, painless, large, small, associated with eachother or not, on the back of the hand, or palms, on the fingers or not and how long have they been there. Have you considered: Treatment with ipl (intense pulsed light)? This can be quite effective for brown spots on the back on the hands.See 1 more doctor answer
Keratosis?: These may represent stucco keratosis which is localized thickening of skin simi, ar to barnacles on a boat. Soak and use peeling cream with urea or lac 5. This will remove surface and improve appearance. If this disturbs you dermatologist can diagnose and remove ith minor procedure.
