Many possibilities: R.I.C.E. Rest, ice, compression, elevation. Immobilization with a cast, splint or brace may be needed in the initial phase of recovery. Oral anti-inflammatory medication may help with the pain and swelling. Once improved, mobilization and physical therapy can help restore strength and function. Healing times may vary.

Cartilage injury: Fractures of the ankle can result in post-traumatic arthritis either imediately or come on years later. Scar tissue and/or cartilage damage is the most common cause. See an orthopedic surgeon for eevaluation which may include plain x-rays, ct scan or mri.

Screw removal time: Usually removal of older fixation screws in the ankle is a simple procedure and recovery is fairly easy as long as your bones are healthy. After the screws are removed, it is prudent to use a CAM walker for about 2 weeks to allow the bones to recover any strength lost from the openings that remain. Pain after removal is usually minimal from the skin incisions.

Possible new fractur: If you are an avid runner or exercise with a lot of impact activities, you may have a new stress fracture. This may or may not be related to your prior fracture, training errors, biomechanics of your legs, etc. See your physician to start with x-rays.

Possibly: I think you're saying you had screws placed for a broken ankle 3 years ago, and now you've sprained your ankle last august and it's still swollen. I actually warn my patients that after bad ankle injuries (in your case, 2 of them), the swelling may actually never go away, but as long as your ankle is functional and painless and strong, I would not worry about some persistent swelling.

Symptomatic: If you don't have gout or active arthritis, then quite likely you have nerve pain. For that, you can try several things: such as liniments, Lidoderm patches, or anticonvulsants--which are good for chronic nerve pain.

Possible soft tissue: I have seen this many times. The ankle can become stiff and painful after a fracture. Sometimes it can be from a tendon or a ligament that was also damaged at the time of the fracture or it can also be from a problems with the cartilage in the joint if the fracture involved the joint line itself. I am biased, but a physiatrist (physical medicine and rehab) can evaluate and help with this.

Possibly: If the initial ankle injury involved the joint surface, pain can persist for a long time. Anytime a joint is involved in injury it is possible for post traumatic arthritis to set in. If no joint involvement then stability and functional issues can create pain, both of which are improved with physical therapy. Bracing and orthotics can provide functional stability and pain reduction also.

Ask your surgeon, or: If your surgeon doesn't think it can be repaired, don't be afraid to ask for a second opinion. Also, was it numb since the surgery or just lately? If lately, you can have a Morton's Neuroma in your foot that causes tingling and pain and can certainly be treated. (It's a painful bundle of nerves.)

CRPS: You might experience some nerve pain. It might due to nerve entrapment or symptoms of complex regional pain syndrome (crps). I recommend you see a pain specialist for evaluation.
