Pain on bottom of big toe when stepping or pressing on it. Almost feels like a stinging pain. What could this be?
This is called sesmoiditis. There are two pea shaped bones on the bottom of the big toe, and with certain activities and shoewear (i.E heels), these bones can get inflamed.

If there is only early spuring, even with a bad joint surface the pain and loss of motion is due to the kissing spurs at the mt-pp joint.
Simple exostectomy can be done and I have had huge sucess with this. Lay up time is minimal and patirnts are happy.

Chip off the old....: It's hard to say without seeing the x-ray. The chip size, location and positioning are all indicators. I would expect that there is a chance that the chip can heal back on to the main bone. The more movement, either by weight bearing of actual toe bending, the less chance of the chip healing.
