A bone island is a small benign sclerotic focus in the bone, but you also mention low density areas in the clavicles, scapula and humeri. Radiology/neurology. Ct scan shows bone, MRI shows soft tissue, so therefore can't indentify disease of skull bones. Intercrainial means misdiagnosis?
Inside bone/tendons.: Bone cysts r contained inside the bone, any where from simple benign @ malignant and need watching. Spurs r at the edges of the bone as in arthritis, or in response 2 tendinitis where a tendon attaches. As patellar tendinitis or where ligaments attach as in heel spurs ( planter fasciitis). For one: You probably should remove the hardware. Increased uptake is informing you there is bone activity. You don't state on what phase. This could indicate a fracture or infection.... The doctor should have a clinical suspicion of what he is looking for. If removing the hardware a bone biopsy can be performed if suspecting infection. ...Read more

Different studies: The meaning of "lobulations" in the bone on an MRI study is uncertain. This may mean an osteophyte, cortical thickening, old trauma, a normal variant, or something else.
