Was diagnosed with prostatitis a couple years back. Occasionally have blood in semen and continuous dull pain in testicle area. Any thoughts?

Prostatitis: The prostate gland accounts for about 2/3 of the volume of ejaculate. Chronic inflammation/infection may reduce this. Treating the inflammatory process and warm sitz baths for about 15-20 minutes twice a day may help, but beyond these measures it is difficult to predict how to increase volume of semen. Yes it does: The prostate is very involved in making up the semen - so if you've been told you have prostatitis then indeed I would expect some abnormalities until your condition is cured. It looks like you're on levofloxacin, which is good medicine for prostate infections - I hope your doc cultured you to be 100% sure. Fistula possible: To confirm- you have a chronic prostatitis for which you take abx. When you ejaculate, semen comes out of your anus- if this is the case, you need to see a urologist, to determine if you have a fistula. Whenever i pass stool (soft or hard) there's always few drops of semen (white/transparent) comes out from urethra. Does this means i've prostatitis?
Urine cultures came back negative, and a semen culture also came back negative. How certain can I be that I don't have Chronic Bacterial Prostatitis?
CBP: That's a very good question. CBP is difficult to treat. The prostate is enveloped in a capsule which makes it difficult for antibiotics to penetrate. In general 3 weeks of antibiotic course is used but there are doctors who treat far longer than this due to symptoms not abating. Prostatitis can be found without physical checkup? Had semen analysis and pus cells 10-15hpf r found. Doctor said this is due to prostatitis.
Physical exam: I would expect a physician who's talking about me having prostatitis to give me a rectal, and probably to check the first bit of urine i pass after the rectal. Diagnosing prostatitis / prostate pain / prostate infection / non-infectious prostate troubles is tricky. If no bacteria, sometimes something as simple as a stool softener helps.
