Bacteria are not found in the blood sample. A routine blood culture only looks for certain types of bacteria germs. Other types of blood cultures can look for viruses, yeast, fungi, tuberculosis, etc...

Caseating granulomas: This sounds like tuberculosis, but may conceivably be systemic fungal infection like Histoplasmosis, rarely lymphoma or sarcoidosis. In India this is probably TB and either should be treated empirically or wait for the results of the biopsied lymph nodes. Sounds as if your doctor is very astute and would heed their advice. See below: There some organisms that are difficult to culture that cause endocarditis such as hacek organisms. I agree with the recommendation for a tee if you have not had one however; lupus can present with libman-sack endocardititis but that should be seen on an echo or transesophageal echo. You may need to see both a rheumatologist and cardiologist if you are not already. ...Read more
