This could be the consequence of a minor bump of the toe of a pressure injury. Without actually seeing it, I guess it could be something else, but I doubt it. Trauma?: Do you remember any trauma to the toenail? A collection of blood underneath the toenail can also appear like this. The safest thing to do is to show this to your friendly dermatologist. While it would be rare of a 20 year old to have a melanoma, it does happen occasionally.
