Renal capsule: Kidney pain is caused classically because of distension or stretching of the renal capsule. This happens with blockages (stones) and infection most commonly. A kidney stone not causing blockage or simple cystitis (uti without kidney involvement) does not typically cause pain. The pain is mediated via the vagus nerve which explains the associated nausea, sweating, etc.

Yes: Pressure on the ureters can cause back pressure against the kidney and cause pain similar to a kidney stone. Pressure on the spinal nerves can cause pain in the back as well. Try a fleets enema, hold for thirty minutes while lying on your left side to liquefy the hard impacted stool and help it to evacuate more easily. Repeat up to three times if needed. Do this the day after you fail a bm.

I have 4 "tiny" stones 2 in each kidney and a very small cyst. I get bad kidney pain sometimes. Can this be caused if stones aren't in ureter?

No: Non-obstructing stones or a small cyst will not cause pain in the kidney. Kidney pain typically results from distention of the renal capsule when the stones move into the ureter. Pain usually occurs during acute obstruction. Cysts in the kidney only cause pain when there is hemorrhage into a cyst or very rarely do they cause blockage of the drainage from the kidney.
