Tough problem: First try all of the conservative options: bracing, therapy, steroid injections, viscosupplemention injections (synvisc, Supartz, (hyaluronic acid) etc). If this fails then sometimes an arthroscopy may help. The science is still out on PRP.

Difficult: Chondromalacia means damaged cartilage, usually from abnormal wear and tear. There is no full cure. Damaged skin leaves a scar and same on inside of body. Physical therapy and long term home therapy and avoiding aggravating activities are appropriate. Finding any aggravating or precipitating musculoskeletal imbalances is appropriate. Surgery is last choice when all fails and intractable pain.

Nonoperative usually: Most chondromalacia of the patella (injured cartilage) is effectively managed with nonoperative measures consisting of activity modification, rest, physical therapy and quadriceps strengthening, non-steroidal anti-inflammatory medications, intrarticular steroid injections and viscosupplementation. At least 6 months of nonoperative treatment is attempted before offering arthroscopic surgery.

Some patella braces and taping can help in some situations, injections with hyaluronic acid liquids, sometimes arthroscopy, sometimes cartilage paste grafts, sometimes patella realignment surgeries. You need a good exam and knee doctor to tell you what you need.

Patella femoral: Best managed with exercise to strengthen your quads to improve tracking, and avoiding deep knee bending activities. Also oral or topical nsaids can be of benefit, as well as a brace. Injection of cortisone or hyaluronic acid also very popular. If all else fails, then scope.

Usually time: True chondromalacia is caused by irritation of the back of the kneecap against other structures of the knee. Usually pain is a main issue. If all you have is cracking, this does need need specific treatment. Surgery is not indicated for this problem altho may help some conditions that are similar. Quad strengthening is key, and tylenol/otc meds often helpful. Ice after exertion, too.

Consider Surgery: Conservative management of Chondromalacia patella includes icing, taping, bracing, and exercise to stabilize your patella. If PT with exercise and icing/taping did not help, an Orthopaedic Surgeon can evaluate you for patella alta or baja, put a camera into your knee to look for a plica, remove damaged cartilage, and give an expert opinion of your biomechanical foot/knee/patellar alignment.

Possibly see ortho: Difficult problem. Especially if your symptoms correlate with your MRI findings. If your pain is behind the knee cap then there is good correlation. Initially nonoperative intervention such as therapy, use of anti inflammatories and patellar brace may be helpful. The possibility of arthroscopic evaluation if all fails may be necessary but still a difficult problem to fix. Supplements controversial.
