Could be not fungal: balanitis could be caused by skin irritation like eczema and best treated by steroid cream or it could be bacterial and best treated with antibiotic or it could be fungal best treated with anti fungal medication oral or topical or both if resistant either they treated the problem with the wrong medicine or you might keep getting back from your partner women could have it without symptom treat both

Cannot diagnose: Without an exam there is no way in which to tell you what this is due to. See your doctor and get evaluated, diagnosed and treated. Good luck.

After treating balanitis will my penis ever look the same? And what is the worst case scenario for long term untreated balanitis?

After treating balanitis will my penis ever look the same? And what is the worst case scenario for long term untreated balanitis?

Yes, it will heal: Balanitis symptoms often evolve over three to seven days. The condition may present as pain, tenderness, or pruritus associated with small erythematous lesions on the glans and/or the prepuce. These lesions can be ulcerated or scaly. Thick foul smelling purulent exudate is often produced. If not properly treated, this process can evolve into a tightening of the prepuce, known as phimosis.
