If new, get checked.: Greetings. Bad cramps can occur before a period starts with PMS. If period is late, it is good to take a home pregnancy test. Bad cramps when pregnant can be a sign of etopic/pregnancy complications and needs care ASAP. If you are having bad cramps that is new or different than what you have encountered in the past, prompt medical care is important to rule out appendicitis or other serious issues.

Oxytocin: The cramping is due to oxytocin release which occurs with breast feeding. It produces cramps very similar to menstrual cramps, but it is extremely unlikely at 4 weeks post-partum and breast feeding that you have ovulated yet and/or going to have any 'period'.
