It varies a lot: Any persistent abdominal or back pain raises a question about Pancreatic Cancer in the middle-aged folks. It does not have to spread outside of the pancreas in order to cause pain. Try reducing the amount you eat. At least reduce the amount you eat at one sitting.

A CT of your pancreas will determine the presence of pancreatic adenocarcinoma (we're not considering pancreatic neuroendocrine tumors in a case like this).
