Without knowing your height, a diagnosis of obesity is impossible, though 209 lbs is typically quite a load for the low back muscles, disc, and joint to handle. Neck/back pain: Lung cancer is unlikely. The symptoms are more likely due to muscle/tendon strain or a disc herniation in the neck or mid back area. Smoking dries out the tissues and can accelerate arthritis. Smoking slows healing from muscle strains and tears too. I had an episode of severe lumbar back pain while bending backward, couldn't get up from bed, worse when pushing myself with my hand or knees. It passed after 5 days of taking naproxen. The MRI showed l have a L5-S1 ~5mm hernia. Could it be from it?

Muscular v disc: This is either muscular issue if only several weeks. If longer, it may be from a disc issue that could be a tear in the disc or a herniation. If muscular, it will get better by about 6-8 weeks and 90% of discs improve over time. Otc meds can help as can heat &/or ice as well as guided exercise & activity modification.
