disc problem: Depends on duration, but chronic pain in that pattern could be caused by a pinched nerve in the neck. A detailed physical and neuro exam, if neck movement or talking on the phone in that arm aggravates symptoms, it is consistent, an MRI might be useful, if it is affecting performance or function. More info: The MRI and CT you speak of were of the head. Did you have a neck imaging test?
Any pinched nerves come from the neck.
Scalp tingling is another separate issue. Need labs and more information.

Consider: Possible inflammatory problem, or maybe a nerve issue involving small fibres, which will not be picked up by your prior studies. Did EMG study brachial plexus?? Were blood tests helpful?
